| Summary, etc. | AMAÇ: Anal fissür tedavisinde lateral internal sfinkterotomi (LİS) ve topikal gliseril trinitrat (GTN) uygulamasının karşılaştırılması. GEREÇ VE YÖNTEM: Polikliniğimize 1997-2000 yılları arasında başvuran ve anal fissür tanısı konulan 30 hasta prospektif olarak çalışmaya alındı. 14 hastaya 6 hafta topikal GTN uygulandı. 16 hastaya topikal GTN uygulanmadan direk olarak LİS uygulandı. Anal fissürlü hastaların copikal GTN ve LİS tedavisine verdikleri cevap, iyileşme ve yan etki oranları açısından irdelenerek karşılaştırıldı. BULGULAR: Topikal GTN uygulanan 14 hastadan 7'si iyileşti. İyileşmeyen 7 hastaya LİS uygulandı. Topikal GTN ile tedavide başarı oranı % 50 idi. Cerrahi uygulanan toplam 23 (16'sı başlangıçta, 7 tanesi topikal GTN tedavisine cevap vermeyen) hastanın hepsi iyileşti. LİS tedavisinde başarı oranı % 100'dü. Topikal GTN grubunda % 50, LİS grubunda ise % 4.5 oranında yan etki görüldü. SONUÇ: Anal fissür tedavisinde, özellikle semptomatik açıdan topikal olarak GTN uygulanabilir. Bu tedavi yöntemi ile kısa süreli bir iyileşme sağlanabilir. Ancak kalıcı bir tedavi için LİS tercih edilmelidir. Anahtar Kelimeler: Anal fissür, Sfinkterotomi, Gliseril trinitrat, Nitrogliserin. SUMMARY PURPOSE: To compare the efficiency of lateral internal sphincterotomy (LIS) and glyceryl trinitrate (GTN) applications. MATERIALS AND METHODS: Between 1997-2000 30 patients who were diagnosed to have anal fissures in our clinic were incluted in the study. 14 patients were treated with topical GTN for 6 weeks. 16 patients underwent directly to LIS without any application of topical GTN. The healing rates, side effects and complications of topical GTN application and LIS were compared RESULTS: Of the 14 patients who received topical GTN therapy 7 patients were waled. The successs rate of GTN therapy was 50%. All 23 patients who undervent LIS (16 LIS patients plus 7 patients who did not respond to GTN) healed. The success rate of LIS was 100%. The side effect rate was 50% in GTN group and 4.5 % in LIS group. CONCLUSION: For the treatment of anal fissures topical GTN can be used especially for symptomatic relief. This method of treatment provides a short period of healing. For a permanet treatment LIS should be the method of choise.
